As I currently launch my new dental practice in Richmond Hill, I invite you to be part of Trout Dental Group. We will remain on the cutting edge of what dentistry has to offer with a specially designed “personalized dentistry program” exclusive to our patients. As we progress, we will continue to keep you informed of all exciting details of our new practice.
As in most new ventures, necessary changes must occur. Effective immediately, I would also like to inform you that I will no longer be seeing patients under my previous dental practice formally known as Smile Innovations or Trout & Copenhaver at Smile Innovations. I will continue to practice dentistry and serve my patients in Richmond Hill under the newly formed “Trout Dental Group” practice name.
I believe that the patient-dentist relationship is fundamental in providing and receiving excellent care. Over the past 21 years that I have practiced and served our patients in Richmond Hill, I have always striven to maintain each patient-dentist relationship based on trust, honesty, respect and a mutual desire to improve your dental health outcome.
In these current circumstances, I believe it is in my patient’s best interest for myself to continue to serve as your dentist under the newly formed Trout Dental Group practice. I therefore encourage those of you who desire to continue on in my care to contact me personally through email (drtrout@troutdentalgroup.com) or by phone at our new office number (912-445-5337) to confirm your intention to remain in my practice patient base. The qualified professional team at Trout Dental Group will then work with you to ensure that your transition from my previously associated practice into my new practice smoothly.
If you prefer to remain with my previously associated practice... you need to do nothing and your records will remain on file there. If you prefer to relocate your records to another practice outside of my previous or new practice, please contact my team and we will assist you with your transition and be pleased to provide a summary of my care while you have been my patient and with your consent will arrange to have a copy of your file transferred. If, at any time in the future, you wish to re-establish contact with our office and allow us to provide you and your family dental care, you will always be welcome.
